NCCHC Foundation Launches Free Reentry Toolkit to Strengthen Continuity of Care

The NCCHC Foundation is proud to launch A Call for Persistence: The NCCHC Reentry Toolkit, a free digital resource designed to help jails and their community partners strengthen reentry programs and continuity of care for people returning to their communities.

Reentry is complex. Individuals leaving correctional facilities may need to maintain access to health care, medications, behavioral health services, benefits, housing, employment, and other essential supports—all at the same time. Without coordinated planning, health gains made during incarceration can be difficult to sustain after release.

With the vast majority of incarcerated individuals eventually returning to their communities, effective reentry planning is an important part of supporting better health and successful transitions.

Practical Guidance for Building Stronger Reentry Programs

Developed with a working group of correctional reentry professionals, A Call for Persistence provides actionable guidance for developing, implementing, and expanding jail reentry programs.

The toolkit includes practical steps, insights from professionals working in jail reentry programs, case studies, and sample tools that can be adapted to meet the needs of jails and communities across the country. Topics include:

  • Continuity of health and behavioral health care
  • Discharge and reentry planning
  • Connections to community-based services
  • Medication access and continuity
  • Benefits and health care navigation
  • Collaboration between correctional and community providers

The toolkit is designed for city and county leaders, jail administrators, correctional health professionals, and reentry and case management staff who are developing or strengthening reentry efforts.

Free and Available to the Field
